Your job is to coordinate everybody who is involved with this project in your community: teachers, principals, parents, children, screeners, and optometrists. You will also need to enter, print, and send information from an iPad, and to make sure that all of the papers and paperwork are handled properly.

Medical tests & privacy

To protect children's and parents' privacy, the law requires that no medical information be seen by anyone at the school, or by anyone else not specifically authorized to see it—including all of us at McMaster and SickKids who will be analyzing the information.

To comply with the law requires these procedures:

  • Every child screened receives a name tag with his name and an ID number.
  • The screeners and optometrists enter their results into forms that are identified solely by that number. Since this information is anonymous, it needs no special protection.
  • You fill in a form lisiting both the child's name and number, plus some other personal information. This form permits associating medical information with individuals, so the law requires it to be kept as private and secure as the records in a doctor's office.
  • Since the children's name tags also permit associating medical information with individuals, the tags must be destroyed at the end of the day.

Normally you will work on iPad with a keyboard. If anything goes wrong with this, you will need to print out a paper version of the form and print labels by hand.

Since the information on the iPad must be kept secure, so must the iPad itself.

McMaster University requires us to send you a formal offer of employment, which you must formally accept, and it then requires you to fill in these forms:

The Hospital for Sick Children requires you to read and sign this form, and to read this page as well. Both of those documents refer to a "note to file" template. If you need one, you will find it here and above under "essential forms."

Health Canada requires you to complete this training module teaching good clinical practice. We shall pay you for four hours' work once you do.

  1. Set the page to "Canada" (top left corner).
  2. Click "Register" (top right corner).
  3. Search for "McMaster University" then continue through steps 2 to 6.
  4. In step 6 enter your normal e-mail address, choose Psychology as the department, and enter your role in research as "Lab Research Staff."
  5. In step 7 choose "Good Clinical Practice (GCP)—Canada" as the course you wish to take and then, on the next screen, select "...Research Personnel."
  6. After your registration is accepted, you will find the course by clicking on the triangle by "McMaster University Courses."
  7. After you finish the course, send the certificate of completion to Sally Stafford .
